What you'll do

You write clinical cases from presentation through workup and plan, the kind a practising PA actually manages, and the reference answers that go with them: differentials weighed, red flags named, reasoning laid out. You also grade model output for diagnostic accuracy, guideline concordance and patient safety, and flag advice that sounds plausible but would harm the patient, the misses only a clinician would catch.

Who fits

• Current PA-C certification (NCCPA) in good standing and an active state PA licence
• 2+ years of clinical practice beyond PA school rotations
• Any specialty: emergency, surgery, primary care, hospital medicine and others
• Someone who can explain why a workup is ordered, not just what the protocol says
• Clear written English, and able to commit at least 10 hours a week

Preferred: depth in emergency, surgical or primary care, precepting, teaching or chart review experience, and a CAQ credential. The form asks for a PA-C credential and a specialty board certification, both optional.

What it pays

The board headlines $100/hr, while the ad text states a range of $80 to $120/hr, set from your experience and paid weekly via Stripe. The ad says the rate is not negotiated down after you start.

Worth knowing

Good:

• Open to every PA specialty, and judged on clinical judgment rather than output volume, according to the ad
• No fixed hours, so it fits around clinical shifts
• No screen recording, keystroke monitoring or idle timers
• Ongoing project work rather than single task batches

Less good:

• A 10 hours a week minimum
• Needs US certification and an active state licence
• A possible unpaid written assessment, which AfterQuery's help centre says cannot be retaken
• Contractor status: no benefits, your own tax to manage
