What you'll do
This is Mercor's broad entry point for physicians: one pool, several concurrent clinical workstreams, and you are matched to them by specialty, availability and interest after onboarding. You are not signing up for one project, and you may be moved as priorities shift.
The work named in the ad:
- Grading criteria: breaking the ideal answer to a clinical question into discrete, checkable criteria, so a model response is scored consistently rather than by impression
- Dialogue evaluation: reading multi-turn clinical conversations and judging accuracy, safety, completeness, hedging and whether escalation advice was right
- Reasoning annotation: recording how you would work a case, including the differentials you considered and rejected
- Output review: catching hallucinated findings, dangerous omissions, false certainty, and advice that is technically correct yet clinically unsafe
- Guidelines: defining edge cases and standards of care in your specialty so a large group of clinicians annotates the same way
- Difficult questions: writing clinical questions that expose the limits of current models
A dialogue evaluation takes roughly 45 minutes; authoring grading criteria runs an hour or more. Each stream sets its own throughput target.
The skill being bought is judgment about ordinary medicine. The failure the ad describes is a routine question answered with misplaced confidence, and spotting it quickly depends on having carried clinical responsibility. Criteria writing is the less familiar part for most physicians: turning "a good answer" into eight or ten items a non-specialist could tick off takes practice, and your first few will likely come back for revision.
Who fits
- MD or DO with a completed residency in any specialty
- Active, unrestricted medical licence in your country of practice
- 3+ years of post-residency clinical experience, practising or previously practising
- Comfortable writing structured clinical rationale a non-specialist reviewer can follow
- Written and spoken English fluency
- At least 20 hours a week, with the ability to concentrate hours when a stream is time-boxed
Preferred: board certification; US licensure and familiarity with US standards of care; primary care, internal medicine, emergency medicine or hospitalist backgrounds (where breadth matters most); prior annotation, AI evaluation, medical education or question writing; grading-criteria or guideline development; and published research or technical writing, with a sample linked.
The listing header says "remote, worldwide", but the ad carries a formal location requirement of the United States, so plan on being US-based.
What it pays
$150 per hour, a flat published figure. At the 20-hour weekly minimum that is about $3,000 a week before tax. It sits well below Mercor's specialist physician listings (the dermatology pool pays $270), which reflects that any specialty qualifies.
Payments are weekly on Stripe or Wise, and you are engaged as an independent contractor.
Worth knowing
Good:
- Any specialty qualifies, so there is no subspecialty gate
- $150/hour with no band to be placed at the bottom of
- Hugely active: Mercor showed 1,624 hires this month when we checked, one of the busiest Mercor listings
- Previously practising physicians qualify
- Criteria and guideline authoring build a skill that transfers to better-paid specialist pools
Less good:
- 20 hours a week is the highest minimum among Mercor's contract physician pools we list
- Lower rate than specialist pools for similar work
- You do not choose your stream, and throughput targets apply
- US-based only, and H-1B and STEM OPT candidates cannot be supported
- Contractor engagement; projects can be extended, shortened or ended early
