- clinical reasoning
- clinical risk assessment
- severity staging
- intervention judgment
- child/adolescent clinical safety expertise
What you'll do
You review clinical case material describing adolescent substance use and document an assessment of risk, severity and intervention urgency. Severity staging runs against established frameworks (DSM-5 SUD criteria and ASAM are named) and produces a recommended level of care. You judge whether proposed and alternative interventions are safe and clinically appropriate, including pharmacological options where they are relevant, and you flag withdrawal, overdose and safeguarding risks that need escalation or immediate action.
A large part of the judgment is developmental: distinguishing experimentation that is normal for the age from a pattern that is clinically significant. You then benchmark your assessments against peer input and published best practice so the set stays consistent.
The written guidance is the deliverable, and the ad states who has to be able to use it: non-specialists, with the rationale explained and the relevant standard or guideline cited. Write for someone without your training.
What you need
- Three to five or more years post-certification working directly with adolescents affected by substance use
- A clinical specialty in addiction medicine, or in child and adolescent psychiatry or psychology with a primary focus on substance use disorders
- An active clinical licence or registration in good standing, with adolescent caseload experience inside the last two years
- Proficiency with adolescent screening and severity staging tools: CRAFFT, S2BI, DSM-5, ASAM criteria
- Familiarity with medication for opioid use disorder (MOUD/MAT) and with withdrawal management protocols for under-18s
- Research-standard written English: native, or IELTS 7.5+ overall with 7.0+ writing, TOEFL 100+, Cambridge C1–C2, or equivalent
These are filed under "preferred qualifications", but the licence and the recent caseload are not the kind of thing a project like this waives.
Nice to have
- Motivational interviewing training
- Adolescent inpatient or crisis experience
- A history of developing guidelines or publishing in youth substance use disorder
What it pays
$45–95/hour. The band is $50 wide with no published basis for where in it you land, and the ad does not say how many cases make up a task or how long one should take.
It carries no compensation-structure section at all, so whether the hourly figure buys your time or is a per-task equivalent is unanswered on the page. Ask before you accept.
Worth knowing
Good:
- $45–95/hour published up front for remote, asynchronous clinical reasoning with no caseload of your own attached
- Your existing addiction or adolescent specialty is the whole qualification; no AI background is asked for
- The work stays inside scope of practice (staging, level-of-care recommendation, escalation) rather than adjacent to it
- Benchmarking against peers and published guidance is written into the brief, which is rare in this market
Less good:
- Three openings. A small specialist panel, not a volume hire, and most applicants will not be placed
- No compensation structure is stated anywhere on the listing
- An active licence and a caseload inside two years rule out anyone recently out of clinical practice
- Contractor engagement: no benefits, no notice period, your own tax to manage
- The output is guidance a non-specialist will act on in youth substance use cases, including where medication is relevant. Worth deciding on deliberately; neither the customer nor the deployment is named
